Rural Health Network of Oswego County

Our Mission

The Rural Health Network of Oswego County will improve the quality, affordability, and availability of health care services in Oswego County by focusing key resources to address specific health care priorities and to strengthen the local health care system. Network staff, leadership and members will accomplish this by coordinating and collaborating existing health services as well as developing new, collaborative, and expanded services based on needs assessment data.  Five major focus areas have emerged as high priority to address health system issues and health care problems and carry out the mission of the Network.  The Network’s committee structure and membership are aligned to address these focus areas:

Central New York Care Collaborative

 The CNYCC is the Central New York Care Collaborative, a collaboration betweeen more than 300 (and growing daily) healthcare and community based service providers located throughout a ten county (Cayuga, Cortland, Jefferson, Lewis, Madison, Oneida, Onondaga, Oswego, St. Lawrence and Tompkins) region in Upstate Central New York.

IMPORTANT MEDICAID UPDATE (May 2014)

There has been a change in state law that applies to Medicaid beneficiaries in your service area who are 21 years of age or older and qualify for Medicare. These beneficiaries are now required to join a managed long-term care plan to receive Medicaid home health care, personal care and other long-term care services.

The American Association on Health and Disability released a Guide to Disability for Healthcare Insurance Marketplace Navigators.

Navigators and other enrollment specialists can use this guide to learn the unique considerations people with disabilities face as they shop for healthcare coverage. For example, someone with a mental health condition or cognitive impairment may have difficulty concentrating or following detailed instructions.

The guide was created as part of the National Disability Navigator Resource Collaborative, an initiative aimed at providing cross-disability information and support to navigators and other professionals providing enrollment support to people with disabilities.

The RHN Funding & Public Policy Committee was charged with creating an electronic reference catalog for the purpose of aiding participating RHN members with a central resource for data and information for grant writing. The Community Commons Resource is described below and provides free access to numerous valuable resources.

Community Commons is an interactive mapping, networking, and learning utility for the broad-based healthy, sustainable, and livable communities' movement. Registered users have FREE access to:

  • Over 7,000 GIS data layers at state, county, zip code, block group, tract, and point-levels
  • Contextualized mapping, visualization, analytic, impact and communication tools and apps
  • Searchable profiles of hundreds of place-based community initiatives (multi-sector collaboratives) working towards healthy/sustainable/livable/equitable communities - funded by government and private philanthropy - complete with text & video narratives on "what's working."
  • Peer learning forums in the "interactive commons" with colleagues exploring similar interests and challenges - hosted by leading national Technical Assistance providers.

The Commons is a democratized learning and innovation platform for:

  • enhancing the reach and impact of currently-funded place based investments;
  • applying an asset-based approach to help grow and sustain impact once the funding is over;
  • and supporting communities of highest burden who heither have grant financing, nor cohorts.

NYS Medicaid Managed Care and Family Health Plus Pharmacy Benefit Information Center:  This website is designed to provide easy access for members and providers looking for information on the drugs and supplies covered by different Medicaid and Family Health Plus health care plans. While pharmacy benefits and participating pharmacies vary among health plans, all plans maintain their own web sites and customer service call centers.
